its the shadow of the photographer not the old man
u used a mask for all gray-thing-area and then excluded colors? (except the man beside the door)
I think he only used lasso tool to draw those people out and desaturated the pic.

Well, at least I can do the basics. I, however, am not a ps pro.
Duplicate layer > adjust levels > desaturate > masking then eliminate the colours that way :)
